p.m. on Tuesday, February 28, 2012 at which time they will be publicly opened and read aloud in
Room 310, City Hall, for Public Works Improvement Contract:

“BAY BEACH PARKING LOT”

in accordance with the plans therefore, the City of Green Bay Standard Specifications and
Construction Standards for Public Works Construction, 1987 Edition and the special provisions
of this contract, all of which are on file in the City Clerk's/Treasurer's Office and the Director of
Public Works Office in the Green Bay City Hall.

This Contract involves the construction of a new gravel parking lot, consisting of approximately
1,950 cubic yards of excavation, 3,720 tons of crushed aggregate base, 35 tons of Type E-1 hot
mix asphalt pavement, and 2,110 square yards of site restoration.

Alternate 1 of this Contract involves 7,115 square yards of grading and site restoration.

The Contract Documents, including plans and specifications, may be examined electronically at
www.questcdn.com and downloaded for a fee of $10.00. Prospective bidders must register with
Quest CDN in order to download documents. Electronic contract documents will also be
available through the Office of the City of Green Bay Director of Public Works, Room 300, City
Hall, 100 North Jefferson Street, Green Bay, Wisconsin 54301 in CD form for a fee of $20.00.
The charges for bidding documents are non-refundable.

Anyone desiring the Contract Documents including plans and specifications by mail shall submit
a separate check made payable to the City of Green Bay in the amount of $5.00 to cover handling
and postage. This check will not be refunded.

The work shall be let in accordance with the following Sections of the Wisconsin Statutes.
1. Section 62.15 regarding public works construction.
2. Section 779.15 regarding lien on contractors.
3. Section 66.0903 regarding municipal wage scale.
4. Section 66.0901(2) regarding proof of responsibility.

All bidders shall provide proof of responsibility on the form furnished by the Director of Public
Works and it shall be filed with the Director of Public Works not less than five (5) days prior to
the time set for opening of bids. Said proof of responsibility shall not be valid if filed prior to
one year of the date of opening bids.

All work performed on this project shall conform with Section 4.12 of the City of Green Bay
Code of Ordinances establishing wage scales on Public Works projects. If this contract has a
copy of a “Prevailing Wage Rate Determination” contained in the Special Provisions, the
Contractor shall incorporate a copy of the determination into all subcontracts. If additional
copies are needed, copies can be obtained from the Office of the Director of Public Works, Room
300, City Hall, 100 North Jefferson Street, Green Bay, Wisconsin 54301. Municipal wage rate
enforcement shall be as provided in Section 66.0903 Wisconsin Statutes.

In accordance with Section 9.16 of the City of Green Bay Code of Ordinances, any corporation,
firm, or individual violating Chapter 133.01 of the Wisconsin Statutes, or any subsequent
amendment thereof, shall upon conviction thereof be thereby disqualified as a bidder on any City
of Green Bay project for a period of three (3) years from the date of such conviction; however,
nothing herein shall be interpreted to preclude such corporation, firm, or individual from
completing any and all contracts he/she may already have with the City at the time of such
conviction, nor shall this ordinance be applied retroactively to convictions occurring prior to the
adoption and publication of this ordinance. This prohibition applies with like force to officers of
convicted corporations, firms, or individuals who thereafter have business interest in new
corporations or business enterprises of whatever kind or description.

All proposals must be submitted on the Bidder's Proposal provided for that purpose and issued to
the specific bidder by the Director of Public Works together with a certified check or a bid bond
equal to at least five (5) but not more than ten (10) percent of the bid payable to the City of Green
Bay as a guarantee that if his/her bid is accepted, he/she will execute and file the contract and a
performance bond in the amount of one hundred percent (100%) of the total bid within ten (10)

This Contract includes the surface preparation and painting of the exterior and interior surfaces and minor repairs for the City of Green Bay’s 2,000,000 gallon Quincy Street elevated water tank.

Contractors are required to perform their own on-site visit prior to submitting the proposal.  Contract Tom Landwehr at (920) 448-3480 at the Green Bay Water Utility for access to the tank.  The tank does not contain an access ladder in the wet interior.

This Contract includes mechanically installed package including two (2) spring tension exhaust hose reels, one (1) 208-230-460/3/60 exhaust fan with silencer, one (1) NEMA 12 control panel with motor starter, high temperature nozzles, safety disconnects, electrical wiring of the fan and control panel and system demonstration.  

A pre-bid meeting will be held at the DPW East Mechanic’s Shop, 1820 Mills St, Green Bay, WI at 9:00 a.m. on Wednesday, August 31, 2011.

The Contract Documents, including plans and specifications, may be examined and obtained from the Office of the City of Green Bay Director of Public Works, Room 300, City Hall, 100 North Jefferson Street, Green Bay, Wisconsin 54301.  Contract Documents will be available electronically at www.questcdn.com for a download fee of $10.00.  Prospective bidders must register with Quest CDN in order to download documents.  Electronic contract documents will also be available through the City of Green Bay for $10.00.  The charges for bidding documents are non-refundable.

Anyone desiring the Contract Documents including plans and specifications by mail shall submit a separate check made payable to the City of Green Bay in the amount of $10.00 to cover handling and postage.  This check will not be refunded.

The work shall be let in accordance with the following Sections of the Wisconsin Statutes.
1.         Section 62.15 regarding public works construction.
2.         Section 779.15 regarding lien on contractors.
3.         Section 66.0903 regarding municipal wage scale.
4.         Section 66.0901(2) regarding proof of responsibility.


All bidders shall provide proof of responsibility on the form furnished by the Director of Public Works and it shall be filed with the Director of Public Works not less than five (5) days prior to the time set for opening of bids.  Said proof of responsibility shall not be valid if filed prior to one year of the date of opening bids.

The Contractor shall give each subcontractor a copy of the “Prevailing Wage Rate Determination” contained in the Contract Documents prior to submittal of the bid.  If a copy of the “Prevailing Wage Determination” has not been provided by the Contractor or additional copies are needed, copies can be obtained from the Director of Public Works, Room 300, City Hall, 100 North Jefferson Street, Green Bay, Wisconsin 54301.

All work performed on this project shall conform with Section 4.12 of the City of Green Bay Code of Ordinances establishing wage scales on Public Works projects.  If this contract has a copy of a “Prevailing Wage Rate Determination” contained in the Special Provisions, the Contractor shall incorporate a copy of the determination into all subcontracts.  If additional copies are needed, copies can be obtained from the Office of the Director of Public Works, Room 300, City Hall, 100 North Jefferson Street, Green Bay, Wisconsin 54301.  Municipal wage rate enforcement shall be as provided in Section 66.0903 Wisconsin Statutes.

In accordance with Section 9.16 of the City of Green Bay Code of Ordinances, any corporation, firm, or individual violating Chapter 133.01 of the Wisconsin Statutes, or any subsequent amendment thereof, shall upon conviction thereof be thereby disqualified as a bidder on any City of Green Bay project for a period of three (3) years from the date of such conviction; however, nothing herein shall be interpreted to preclude such corporation, firm, or individual from completing any and all contracts he/she may already have with the City at the time of such conviction, nor shall this ordinance be applied retroactively to convictions occurring prior to the adoption and publication of this ordinance.  This prohibition applies with like force to officers of convicted corporations, firms, or individuals who thereafter have business interest in new corporations or business enterprises of whatever kind or description.

Sealed bids shall be delivered or addressed to the City Clerk/Treasurer, Room 106, City Hall, 100 North Jefferson Street, Green Bay, Wisconsin, 54301.

The City of Green Bay, Wisconsin reserves the right to reject any or all bids and to waive any informalities in bidding.

No bids shall be withdrawn after the opening of bids without the consent of the City of Green Bay, Wisconsin for a period of sixty (60) days after the scheduled time for closing bids.

All proposals must be submitted on the Bidder's Proposal provided for that purpose and issued to the specific bidder by the Director of Public Works together with a certified check or a bid bond equal to at least five (5) but not more than ten (10) percent of the bid payable to the City of Green Bay as a guarantee that if his/her bid is accepted, he/she will execute and file the contract and a performance bond in the amount of one hundred percent (100%) of the total bid within ten (10) days after the award of the contract.  Only the Proposal Pages, Bid Bond or Certified Check, and


any Addenda issued shall be submitted.  Only proposals submitted by firms appearing on the City of Green Bay list of plan holders will be opened.

In case the successful bidder shall fail to execute such contract and performance bond, the amount of the check or bid bond shall be forfeited to the City as liquidated damages.
